Qualifications

• Active RN license in the state of employment (OH).
• Graduate of an accredited nursing program.
• Minimum 3 years direct patient care, incl. 2 in hospice/home care.
• Knowledge of hospice principles, Medicare/Medicaid, JCAHO/ACHC standards.
• Ability to lead a diverse care team and handle emotional needs.

Responsibilities

• Oversee and direct patient care to ensure high quality hospice services.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings; present updates and coordinate care plans.
• Mentor and support associates; promote compliance and development.
• Ensure regulatory standards and company policies are followed; drive performance improvements.
