Job summary

Affinity Hospice Management LLC seeks a licensed Nurse Practitioner to provide comprehensive hospice care in Ohio homes and communities. Under physician supervision, assess patients, direct plans of care, and collaborate with the interdisciplinary team to ensure comfort and quality of life.

The NP will perform face-to-face assessments, evaluate eligibility, coordinate with families and healthcare providers, and document clinical findings while adhering to HIPAA and state regulations.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Has a current DEA license.
• Current nursing licensure in State and CPR certification.

Responsibilities

• Provide medical care, palliate and manage terminal illness under supervision of a collaborating physician.
• Complete history and physical examinations, order/interpret tests, and document findings.
• Develop an individualized plan of care, prescribe meds/treatments, coordinate hospice services.
