Job summary

Affinity Hospice Management LLC is seeking a Nurse Practitioner to provide comprehensive hospice care within patients' homes under physician supervision. You will conduct assessments, develop care plans, prescribe treatments, and collaborate with families and the hospice team to ensure quality end-of-life care.

You will perform face-to-face visits, verify eligibility for hospice, coordinate with the medical director, and educate families to support comfort and dignity at the end of life.

Qualifications

• Graduation from an accredited School of Nursing and NP program.
• Current state NP licensure and CPR certification.
• Master’s degree as Nurse Practitioner with hospice/palliative care experience.
• DEA license, if required by state regulations.
• Accreditation by a certifying body for advanced practice nursing.

Responsibilities

• Provide medical care and symptom management for terminally ill patients.
• Complete history and physical examinations and document findings.
• Develop individualized care plans and coordinate hospice services.
• Perform hospice face-to-face visits to assess eligibility and recertification.
• Collaborate with the hospice inter-disciplinary team and physicians.
• Educate patients and families on care options and end-of-life planning.
• Maintain accurate clinical documentation and ensure HIPAA compliance.
