Job summary
Ohio Living seeks a licensed Nurse Practitioner to deliver preventive, palliative, and educational services in home health and hospice settings across Ohio. You will use standard clinical-financial systems to ensure compliance with state and federal guidelines while supporting patient health and wellness.
The role requires two years of home health/hospice experience, OH NP license, prescriptive authority, and strong EMR and MS Office proficiency.
Qualifications
- Current Ohio license to practice as a Nurse Practitioner.
- Current prescriptive authority.
- Two years of home health or hospice experience.
- Proficiency with Windows, Microsoft Office, and the internet.
- EMR systems experience preferred.
- Must read, write, speak, and understand English.
- Valid driver’s license and acceptable driving record.
- Must be insurable under professional liability and crime coverage policies.
Responsibilities
- Conducts comprehensive medical and social histories and physical examinations.
- Treats acute illnesses, chronic conditions, and provides related care.
- Orders and interprets lab tests and x-rays.
- Provides information and counseling on health maintenance and palliative care.
- Refers patients to other health professionals as needed.
- Communicates with staff, patients, and families to promote outcomes.
- Documents accurately and timely per regulations.
- Participates in quality improvement projects.
- Demonstrates commitment to excellent customer service.
- All other duties as assigned.
