Job summary

Affinity Health Management seeks a Nurse Practitioner to provide comprehensive hospice services under physician supervision. You will assess and manage terminal illnesses, develop individualized care plans, and coordinate with families, the hospice team, and community providers.

Responsibilities include conducting history and physicals, prescribing treatments, and performing face-to-face eligibility assessments. Strong communication and HIPAA/compliance knowledge are essential.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license.
• Current nursing licensure in State and CPR certification.
• Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.
• Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
• Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Provides medical care, palliates and manages terminal illness under physician supervision.
• Completes history and physical examinations and documents findings.
• Develops and implements an individualized plan of care; prescribes medications.
• Completes hospice face-to-face visit requirements to assess eligibility.
• Advices hospice medical director regarding certification/recertification.
• Reviews clinical information and coordinates patient care with the team.
• Ensures symptom control through team collaboration.
• Educates patients and families about care and end-of-life planning.
• Establishes relationships with staff and community referral sources.
• Maintains compliance with Medicare/Medicaid and HIPAA requirements.
