Job summary

Affinity Hospice Management LLC seeks a Nurse Practitioner to deliver comprehensive hospice care under physician supervision. You will evaluate health histories, diagnose conditions, and direct plans of care in home/community settings, coordinating with families and the interdisciplinary team.

The role requires an NP with strong clinical and communication skills, current state licensure, CPR, and DEA where applicable. Hospice-specific experience and CHPN certification are highly valued.

Qualifications

• Must have nursing degree and accredited NP program.
• DEA license if state regulations require it.
• Current nursing licensure in state and CPR certification.
• Master's degree with 1+ year NP experience and hospice/palliative care.
• Certification by a certifying body for advanced practice nursing.
• CHPN or related certification preferred (pediatrics/geriatrics).
• Excellent observation, verbal and written communication, problem solving and math skills.
• Physically able to lift/move patients; prolonged walking/standing.
• Licensed driver with insured automobile.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history and physical exams; order/interpret tests; document findings.
• Develop and implement individualized care plans; prescribe meds or therapies; coordinate services.
• Complete hospice face-to-face visits for eligibility reviews and recertification with physician input.
• Make recommendations on eligibility for hospice certification/recertification.
• Review clinical information; coordinate care with the hospice interdisciplinary team.
• Ensure symptom control through collaborative care planning.
• Develop treatment protocols with the collaborating physician; discuss non-standard plans.
