Job summary

Ohio Living is seeking a Nurse Practitioner to provide preventive, palliative, and educational services to patients in our home health and hospice programs, using our comprehensive clinical-financial system to ensure compliance. You will take histories, perform exams, order tests, prescribe medications, and collaborate with staff to promote patient outcomes.

A Ohio NP license and Prescriptive Authority are required, with at least two years in home health or hospice.

Qualifications

Must hold an active Ohio Certified Nurse Practitioner license.
Current Prescriptive Authority (or Externship to Prescribe).
Minimum of 2 years experience in home health and/or hospice.
Proficiency with Windows, Microsoft Office, and the internet is required.
Proficiency with electronic medical records (EMR) is preferred.

Responsibilities

Care Management - 100% involvement in preventive, palliative, and educational services.
Conduct comprehensive histories and physical exams for diverse patient populations.
Order and interpret labs and imaging; prescribe medications as indicated.
Communicate with patients, families, and staff to optimize outcomes.
Complete documentation accurately while complying with regulations.
