Job summary

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current APRN license in Ohio with DEA registration.
• At least 1 year of hospice or palliative care experience preferred.
• Excellent communication, documentation and clinical skills.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Complete history and physical exams; interpret lab and diagnostic tests.
• Develop individualized plans of care; prescribe medications and treatments.
• Evaluate hospice eligibility and coordinate with the interdisciplinary team.
• Educate patients and families on end-of-life care and comfort measures.
• Document clinical notes accurately; maintain medical records.
• Communicate with the team and participate in hospice group meetings.
