Job summary

Affinity Health Management seeks a Nurse Practitioner to provide comprehensive hospice services in Ohio under physician supervision. You will assess health histories, diagnose, manage care plans, and coordinate with families and the interdisciplinary team to deliver hospice care in patients' homes and communities.

Qualifications

• Must have graduation from an accredited school of nursing and an accredited NP program.
• Current DEA license.
• Current nursing licensure in state and CPR certification.
• Master's degree with minimum 1 year NP experience and hospice/palliative care experience.
• Certification in a specialist area (e.g., CHPN) preferred.

Responsibilities

• Provide medical care and palliative management of terminal illness under physician supervision.
• Perform history and physical exams; order/interpret tests and document findings.
• Develop individualized plan of care; prescribe medications and coordinate hospice services.
• Complete hospice face-to-face visits to assess eligibility.
• Communicate with and educate patients/families; ensure safe environment and continuity of care.
