Job summary

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Has a current DEA license
• Current nursing licensure in State and CPR certification.
• Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.
• Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
• Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Provides medical care and manages terminal illness under physician supervision.
• Completes history and physical examinations, orders/ interprets labs, documents findings.
• Develops individualized plan of care, prescribes medications, coordinates hospice care.
• Completes hospice face-to-face visit requirements to assess eligibility; provides information to medical director.
• Makes recommendations to the hospice medical director regarding eligibility for certification/recertification.
• Reviews clinical information and oversees patient care with the interdisciplinary team.
• Assures symptom control through collaboration with team.
• Reviews and develops treatment protocols with physician; consults when outside standard practice.
• Communicates with interdisciplinary group to plan care; educates patients and families.
• Establishes relationships with staff and community referral sources to facilitate program growth.
