Job summary

Affinity Health Management is seeking a Nurse Practitioner to deliver comprehensive hospice services under physician supervision. The NP will evaluate patients, develop care plans, prescribe medications, and coordinate with families and the hospice team to ensure high‑quality end‑of‑life care.

The role requires an NP license, DEA, and a Master’s degree in Nursing, with experience in hospice or palliative care preferred.

Qualifications

• Graduation from an accredited School of Nursing required.
• Current DEA license and nursing licensure in state with CPR certification.
• Master's degree with at least 1 year NP and hospice/palliative care experience preferred.

Responsibilities

• Provide hospice care under physician supervision, including evaluation and plan of care.
• Complete history/physical exams and interpret tests to inform treatment.
• Prescribe medications/treatments and coordinate care with patients and families.
• Conduct hospice face-to-face visits to assess eligibility with the hospice team.
• Communicate and collaborate with interdisciplinary team to update care plans.
