Job summary

Affinity Hospice Management LLC is seeking a Nurse Practitioner to provide comprehensive hospice care under physician supervision. You will evaluate patients, develop and implement plans of care, prescribe medications, and coordinate with families and the care team to support home-based services.

The role requires a current NP license, state nursing licensure, and DEA registration. You will perform face-to-face assessments, ensure hospice eligibility, and participate in interdisciplinary group

Qualifications

• Excellent observation, verbal and written communication skills.
• Ability to develop individualized plans of care.
• Strong coordination with the hospice interdisciplinary team.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Complete H&P exams, document findings, order/interpret tests.
• Develop and implement a plan of care, prescribe meds, coordinate services.
• Perform hospice face-to-face visits to assess eligibility.
• Advise on certification/recertification with hospice medical director.
• Review information and coordinate patient care with the team.
• Ensure symptom control through interdisciplinary collaboration.
• Develop protocols for treatment in consultation with physician.
• Consult with physician when plans exceed standard practice.
