Job summary

Affinity Health Management seeks a Nurse Practitioner to provide comprehensive hospice services under physician supervision. You will evaluate health history, develop care plans, prescribe medications, and coordinate care with patients and families in home and community settings.

Responsibilities include conducting histories and physicals, coordinating interdisciplinary care, ensuring compliance with hospice regulations, and supporting quality improvement initiatives.

Qualifications

• Graduation from an accredited School of Nursing and a valid Nurse Practitioner program.
• Current DEA license if state regulations require.
• Active state RN licensure and CPR certification.
• Advanced practice nursing certification preferred; CHPN or related is a plus.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history and physical exams and interpret diagnostic results.
• Develop individualized plans of care and prescribe medications or treatments.
• Complete hospice face-to-face visits to assess eligibility per policy.
• Consult with hospice medical director on eligibility for certification/recertification.
• Coordinate with interdisciplinary team to oversee patient care.
• Ensure symptom control and safe patient care in collaboration with team.
• Develop treatment protocols and propose intervention options based on literature.
• Consult with collaborating physician when plans exceed standard practice.
• Communicate plans with patients, families, and care teams; educate as needed.
• Establish relationships with community referral sources to support program growth.

Education

Master's degree in Nurse Practitioner program

DEA license (if applicable)
