Job summary

Adoration Health Hospice Nurse Practitioners provide compassionate hospice care by evaluating and managing terminal illness, and developing personalized plans under supervision of a collaborating physician. You will manage patient care, order and interpret assessments, and contribute to interdisciplinary discussions for optimal comfort and dignity at the end of life.

The role requires an NP credential and current nursing license, plus DEA where applicable, CPR, and a Master’s in Nursing.

Qualifications

• Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.

Responsibilities

• Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
• Completing history and physical examinations by evaluating the patient's medical condition and health history
• Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
• Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
• Completing and maintaining accurate clinical notes
• Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
• Ensuring compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned
