Job summary

Affinity Hospice Management LLC seeks a Nurse Practitioner to deliver comprehensive hospice care under physician supervision, conducting history/physicals, eligibility assessments, and care coordination with the hospice team in patients’ homes and communities.

The role requires an accredited NP program, current nursing licensure, CPR, and ideally hospice/palliative care experience. Strong communication and physical stamina are essential; a valid driver’s license is preferred.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current nursing licensure in State and CPR certification.
• Master's degree with NP experience; hospice or palliative care experience.
• DEA license if applicable to state regulations.
• Acquisition of certification in a specialist area (CHPN, Pediatrics, Geriatrics) preferred.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.
• Excellent observation, verbal and written communication, problem solving, and nursing skills.

Responsibilities

• Provides medical care and palliates/manage terminal illness under physician supervision.
• Completes history and physical examinations and documents findings.
• Develops an individualized plan of care and prescribes medications.
• Assess hospice eligibility with interdisciplinary team involvement.
• Coordinate hospice care services and obtain consultations/referrals.
• Communicates with the interdisciplinary group to review and revise care plans.
• Educates patients/families on preventative care and end-of-life issues.
• Maintains HIPAA compliance and accurate clinical documentation.
• Maintains equipment, attends team meetings and supports program growth.

Education

Master's degree with NP

Current nursing licensure

DEA license (if applicable)

CPR certification

Hospice or palliative care experience

Certification in a specialist area (CHPN, Pediatrics, Geriatrics)

Accreditation from certifying body for advanced practice nursing

Excellent observation, verbal and written communication

Licensed driver with insured automobile
