Job summary

The NP will perform exams, diagnose conditions, order tests, and participate in discharge planning. The ideal candidate holds a Master’s in Nursing, California NP license, 1–2 years of NP experience, and strong communication skills.

Qualifications

• Master’s degree in Nursing required and California NP licensure in good standing.
• 1–2 years of clinical NP experience or equivalent preferred.
• Advanced clinical knowledge for patient care and treatment.
• Proficiency with Microsoft Office tools and ability to learn new software.

Responsibilities

• Provide comprehensive NP care in home health for hospice/palliative patients.
• Coordinate multidisciplinary care from admission to discharge.
• Perform history and physical exams, diagnose and treat medical problems.
• Order and interpret diagnostics, perform minor procedures.
• Assist with discharge planning and care coordination.
