Job summary

Hoag Hospital is seeking a Nurse Practitioner for Palliative/Hospice care in a home health setting. The NP will coordinate care from admission to discharge, perform exams, diagnose and treat, and manage orders and discharge planning in collaboration with the multidisciplinary team.

The role emphasizes patient safety, clear communication, and adherence to clinical standards. Work aligns with home health operations and local travel as needed.

Qualifications

• Master's degree in Nursing required.
• 1-2 years of clinical NP experience.
• Current California NP license with furnishing license and DEA number.
• BLS and ACLS certifications; national NP board certification.
• Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Visio).

Responsibilities

• Provide NP care in home health to support multidisciplinary team.
• Conduct history and physical exams, diagnose and treat conditions.
• Order/interpret diagnostics; perform minor procedures; discharge planning.
• Ensure patient safety and high-quality care; maintain clear documentation.
• Collaborate with Hoag Hospital and affiliates across care settings.
