Job summary

Bridge Home Health & Hospice is seeking an experienced Nurse Practitioner to provide comprehensive home-based medical care within the California hospice program. You will assess, diagnose, and manage patient conditions, coordinate care with the primary team, and ensure compliance with all regulatory requirements.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current nursing licensure in California and CPR certification.
• Master's degree with NP credential and at least 1 year NP experience.
• Certification in Hospice and Palliative Care preferred.
• 2 years Hospice experience preferred.
• Excellent observation, verbal and written communication skills.
• Must possess a California Driver’s License and reliable vehicle.
• Collaborative agreement with a physician required.

Responsibilities

• Provides medical care under the supervision of a collaborating physician and complies with state Nurse Practice Act.
• Completes history and physical examinations, orders and interprets tests, and documents findings in medical records.
• Develops individualized care plans, prescribes medications or treatments, and coordinates services.
• Communicates and collaborates with the care team to update the patient’s plan of care.
• Educates patients and families, ensuring continuous, safe home-based care.
• Participates in on-call rotation as needed and maintains HIPAA-compliant documentation.
