Continuum Care Hospice is seeking a Nurse Practitioner to provide comprehensive hospice services under physician supervision. The NP will assess health history, diagnose conditions, develop plans of care, and prescribe treatments for terminally ill patients in home and community settings.

The role requires collaboration with the hospice interdisciplinary team, completion of face-to-face assessments for eligibility, and ongoing coordination with patients, families and other healthcare

Formación

• Registered professional nurse with advanced graduate education and clinical training
• Supervision of a collaborating physician
• Evaluates hospice eligibility in consultation with the hospice interdisciplinary team

Responsabilidades

• Provides medical care and manages terminal illness in compliance with the Nurse Practice Act under the supervision of a collaborating physician.
• Completes history and physical examinations and orders/interprets tests.
• Develops an individualized plan of care and prescribes medications or treatments.
• Coordinates hospice care services with patients, families and other healthcare practitioners.
• Performs hospice face-to-face visit requirements and documents eligibility.
• Maintains accurate clinical notes and documentation in the medical record.
• Educates patients and families on preventive care and medical issues with the interdisciplinary team.
