Job summary

Qualifications

• Master’s degree in Nursing required.
• 1–2 years of clinical NP experience preferred.

Responsibilities

• Provide comprehensive NP care in Home Health for patients from admission to discharge.
• Perform history and physical exams, diagnose and treat medical problems.
• Order and interpret tests, perform minor procedures, facilitate discharge.
• Maintain patient safety and documentation according to policies.
• Communicate effectively with patients, families, and care teams.
