Job summary

Allied Services seeks an experienced Registered Nurse to oversee a Hospice/Palliative patient caseload under the Clinical Director. You will assess patient needs, develop the IDT plan of care, implement interventions, and reevaluate to meet comfort and quality goals.

You will supervise Hospice Aides/Homemakers and coordinate with the IDT for optimal patient outcomes. The role requires strong communication, empathy, and the ability to work within an interdisciplinary team in diverse patient homes

Qualifications

• Graduate of an accredited Nursing Program.
• Bachelor’s Degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.

Responsibilities

• Perform initial and comprehensive nursing assessments for Hospice/Palliative patients and document findings.
• Develop and update the interdisciplinary care plan in collaboration with the IDT.
• Provide skilled nursing interventions and responsive care to patients, families, and caregivers.
• Supervise Hospice Aides/Homemakers and ensure quality of care.
• Collaborate with physicians to coordinate medical and nursing plans of care.
• Maintain timely and accurate clinical documentation for all patient encounters.
