Job summary

Advocate Health Care is seeking an experienced Registered Nurse to provide comprehensive hospice and end-of-life care in various settings. You will coordinate patient and family care, educate on disease processes and care plans, and supervise nursing staff within an interdisciplinary team.

You will review caseloads for appropriate hospice level of care, administer prescribed treatments, document thoroughly, and collaborate with physicians to adjust plans as needed.

Qualifications

• Experience in hospice or palliative care preferred.
• Strong communication and assessment skills.
• Ability to educate patients and families about disease processes and care plans.
• Proficiency with electronic health records and documenting care.

Responsibilities

• Identify and coordinate patient/family care for terminally ill patients in hospice settings.
• Educate patients and families on disease processes, trajectory, and care plans.
• Review caseload to ensure appropriate level of hospice care and refer to team members as needed.
• Administer medications and treatments as prescribed by the physician.
• Lead IDG to develop and implement each patient’s Plan of Care.
• Document in the electronic health record according to policies and guidelines.
• Assess patient response to treatments and adjust care as needed.
• Supervise Home Health Aide and licensed practical nurse visits.
