The RN, Case Manager at Chapters Health provides and coordinates expert home-based hospice and palliative care, prioritizing patient comfort and family support within the IDT. You will assess needs, develop and adjust the Plan of Care, and ensure timely physician orders and documentation.

• RN license in state where employed is required.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Alzheimer's/Dementia training certification (PACE) may be required in some locations.
• Experience with EMR/EHR systems.
• Valid driver's license and auto insurance per policy; reliable transportation.

Responsabilidades

• Provides and manages direct care to patients and families as part of IDT; incorporates psychosocial, spiritual and physical care.
• Coordinates the Plan of Care with IDT to reflect evolving patient needs.
• Educates patients and families about disease, palliation, safety and care options.
• Home visits to assess safety, medication compliance, and ability to live at home.
• Communicates changes in patient condition to IDT and physicians; documents appropriately.
• Participates in IDT meetings; presents concise reports and supports referrals to service lines.
• Supervises home health aides as requested and supports emergency coverage as needed.
