Job summary

Paycom - ATS is seeking an experienced RN to manage the interdisciplinary team (IDT) in Tallahassee, FL. This full-time role focuses on coordinating daily IDT staffing, triaging clinical calls, and ensuring continuity of care for hospice patients.

The RN will supervise IDT members, participate in 24/7 patient care, and promote BBH policies while supporting staff development and education for symptom control and community resources.

Qualifications

• Current RN license in Florida.
• Must maintain current Florida RN licensure.
• Minimum of two years hospice experience preferred.
• Oncology or home health nursing experience with at least one year acute care.
• Must possess valid driver's license and reliable transportation.

Responsibilities

• Determine daily IDT staff utilization.
• Triage clinical calls to IDT members.
• Provide coverage to IDT teams to ensure continuity of care.
• Participate in 24/7 patient care coverage as needed.
• Participate in clinical on-call staffing.
• Promote adherence to BBH policies and procedures.
• Obtain reports and monitor patient and family needs; initiate intervention.
• Educate IDT members on symptom control and resources.
• Administer direct care to select patient to maintain clinical competence.
• Oversee IDT operations and schedules.
• Coordinate weekly IDT meetings and maintain Kardex.
• Collaborate with departments to meet patient/family needs.
• Communicate clearly and provide feedback.
• Support professional development and staff certifications.
• Engage in leadership development and professional organizations.
• Represent BBH at community activities.
