Job summary

Big Bend Hospice is seeking a full-time Registered Nurse to manage clinical resources for the Interdisciplinary Team (IDT) in Tallahassee. You will determine daily staff utilization and act as a role model, mentor, and resource for the team.

This role requires Florida RN licensure, with hospice and oncology/home health experience preferred. You will participate in 24/7 patient care coverage and coordinate with On-Call Nurse and Admissions to meet patient and family needs.

Qualifications

• Current RN license in Florida.
• Minimum of two years hospice experience preferred.
• Oncology or home health nursing experience preferred with at least one year acute and medical surgical.
• Must maintain current Florida RN licensure.

Responsibilities

• Primary clinical and administrative resource for the Interdisciplinary Team IDT.
• Determines the daily utilization of IDT staff.
• Appropriately triage clinical calls to IDT members.
• Provide coverage to team as needed and per patient acuity to ensure continuity of care.
• Participates in 24-hour/day, 7-day/week patient care as needed.
• Participates in clinical on-call staffing as needed.
• Promotes adherence to BBH policies and procedures and adapts care as indicated.
• Obtains reports and monitors ongoing assessments and initiates interventions with IDT.
• Provides education to IDT members regarding symptom control and case management.
• Enhances IDT staff practice as a role model, mentor, instructor and resource.
• Maintains daily IDT operations and schedules on-call staffing.
• Maintains Kardex and prepares IDT agenda; collaborates with other departments.
