Job summary

Qualifications

• Medical degree (MD or DO) from an accredited medical school.
• Current and unencumbered license to practice in the state.
• Membership in good standing on the medical staff of at least one contracted hospital.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive, face-to-face assessments with patients and manage symptoms.
• Oversee admissions, discharges, certifications, recertifications, and medication management.
• Lead goals of care discussions with patients and families when appropriate.
• Coordinate care with other providers to ensure continuity and quality of hospice services.
• Maintain accurate medical records and participate in quality improvement activities.
