Qualifications

• Medical degree (MD or DO) from an accredited medical school.
• Current and unencumbered license to practice medicine in the specified state.
• Membership in good standing on the medical staff of at least one contracted hospital.
• Current CPR certification.
• Valid driver's license and reliable transportation with agency-required liability insurance for home visits.

Responsibilities

• Conduct comprehensive face-to-face assessments and provide palliative care.
• Oversee admissions, discharges, certifications, recertifications and medication management.
• Assure appropriate evaluation, certification, and recertification of terminal prognosis at the time of admission and at the end of each certification period.
• Lead goals of care discussions with patients and families.
• Coordinate care with other healthcare providers to ensure seamless transitions and continuity of care.
• Provide on-call coverage for medical emergencies and patient referrals.
• Maintain accurate medical records and ensure regulatory compliance.
• Supervise Nurse Practitioners per state requirements.
• Educate patients and families about hospice philosophy and end-of-life care.
• Collaborate with Hospice Medical Director and care center physicians.
• Perform other duties as assigned.
