Qualifications

Responsibilities

• Responds to after-hours patient census and referrals.
• Provides urgent clinical and psychosocial interventions.
• Maintains patient safety and manages pain and symptoms.
• Documents care thoroughly and timely.
• Communicates with physicians and IDG for continuity of care.
• Travels to patient homes as needed.
• Participates in mandatory meetings and maintains professional knowledge.
