Two years of hospice, home health, or related nursing experience preferred.
Must hold a valid driver's license and have access to a insured vehicle.
Ability to document care promptly and maintain patient confidentiality per HIPAA.

Responsibilities

Respond to Hospice census and referrals after hours, every other weekend.
Conduct urgent physical and psychosocial assessments and determine interventions.
Provide pain and symptom management and safe nursing care.
Document patient care thoroughly and timely, including changes in status.
Collaborate with physicians, IDG, and other providers to coordinate care.
Maintain professional knowledge through continuing education and in-services.
Assist with after-hours admissions and communicate patient needs clearly.
