Job summary

Affinity Hospice Management LLC is seeking an On-Call Hospice RN to respond to after-hours patient census and family inquiries. The role emphasizes urgent clinical assessment, pain and symptom management, and collaboration with the Hospice Medical Director and clinical team.

The position requires an RN license (Florida) and means visiting patients in home settings, driving to assignments, and maintaining timely documentation. Two years of hospice or related experience is preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Maintains current unencumbered RN license in Florida.

Responsibilities

• Respond to after-hours census and referrals during nights, weekends and holidays.
• Perform urgent physical and psychosocial assessments and determine interventions.
• Provide pain and symptom management.
• Document care thoroughly and timely in the medical record.
• Collaborate with physicians and the care team to ensure continuity of care.
• Communicate patient needs to the IDG and vendors after hours.
• Assist with admissions and coordination of services as needed.
• Maintain patient safety and home environment during visits.
