Job summary

St. Croix Hospice is seeking an experienced RN Clinical Supervisor to coordinate patient care across home hospice teams in Missouri. The role focuses on directing care plans, reviewing cases, and ensuring compliance with regulatory standards.

You will lead interdisciplinary groups, mentor clinicians, participate in case conferences, and support ongoing quality improvement while maintaining a strong patient- and family-centered approach.

Qualifications

• RN license in state(s) of practice required.
• Graduate of an accredited school of nursing; associate degree required.
• Proven ability to work within an interdisciplinary setting.

Responsibilities

• Reviews case referrals and determines hospice/home care needs.
• Assigns appropriate hospice personnel to cases.
• Conferences with Medical Director or Hospice Physician on eligibility and care.
• Reviews and evaluates cases; guides clinicians to improve care quality.
• Attends case conferences and coordinates interdisciplinary team involvement.
