Job summary

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RN Case Manager provides day-to-day patient care, administers medications, maintains records, and coordinates with physicians and interdisciplinary teams to enhance comfort and quality of life for patients in home settings.

Qualifications

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their RN license.
• Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Responsibilities

• Provides day-to-day patient care and administers medications as prescribed.
• Leads the patient’s hospice care team and develops, implements, and revises the Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff.
• Coordinates care with physicians, interdisciplinary teams, and community health partners.
• Maintains accurate clinical documentation and HIPAA compliance.
• Ensures a safe environment and supports daily living activities for patients.
