Job summary

Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, is seeking a Hospice Registered Nurse to deliver skilled nursing care to hospice clients in their homes or community settings.

Qualifications

• Graduate of an Associate’s or diploma program in nursing.
• One year nursing experience or one year nursing experience in a specialty.
• License to practice in the State of Connecticut.
• Graduate of NLN approved BSN program (preferred).
• Medical/surgery or home care experience preferred.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts complete nursing assessment based on referral data, client/family interview, and observation of patient health status.
• Develops a plan of care with goals, discharge planning, and resource utilization.
• Facilitates bereavement follow-up through timely documentation and coordination of care.
• Directs LPNs and Home Care Aides; supervises the plan of care.
• Teaches patients and caregivers; educates on signs and symptoms of death when appropriate.
