Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care to hospice clients in their homes, ensuring high-quality, cost-effective care. You will assess patient needs, coordinate with families and the interdisciplinary team, and modify plans as conditions change.

Qualified candidates hold an Associate’s or Diploma in Nursing with CT licensure and at least one year of nursing experience; a BSN is preferred.

Qualifications

• Graduate of an Associate’s or diploma program in nursing.
• One year nursing experience OR one year nursing experience in a specialty.
• License to practice in the State of Connecticut.
• BSN preferred (NLN approved).
• Medical/surgery or home care experience preferred.

Responsibilities

• Provides skilled nursing care to hospice clients in their homes or community health settings ensuring high-quality, outcome-based care.
• Reviews referrals, and initiates contact with client and family.
• Conducts complete nursing assessments and develops a plan of care with client and disciplines.
• Manages care, facilitates bereavement follow-up, and documents care plans.
• Directs LPNs and Home Care Aides in implementing the plan of care; coordinates with the physician.
