Job summary

Elara Caring is seeking a Hospice Nurse Practitioner (PRN) in Springfield, IL to lead hospice care in patients' homes. The role emphasizes comprehensive assessment, symptom management, and collaboration with the interdisciplinary team to optimize the Plan of Care.

The candidate should have active NP licensure in Illinois, an MSN, and DEA certification where allowed. Experience in hospice/palliative care is preferred and a valid driver's license is required for home visits.

Qualifications

• Master’s degree in nursing required.
• Nurse Practitioner license/certification required.
• Active DEA certification if state allows prescribing controlled substances.

Responsibilities

• Perform routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs, document findings, and collaborate with physicians and care team.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs and prevent hospitalizations by addressing home care requirements.
• Maintain accurate records and CMS-compliant documentation.
