Job summary

Elara Caring is seeking a Hospice Nurse Practitioner in Tinley Park, IL for a part-time role. You will conduct routine and urgent face-to-face recertifications, assess patient needs, and work with the care team to manage symptoms and support the Plan of Care in the home setting.

The position requires an NP license, MSN and certification, DEA if applicable, and at least one year of clinical nursing experience, with hospice experience preferred.

Qualifications

• Current licensure in the state of practice as a Nurse Practitioner.
• Master’s degree in nursing and certification through a national Nurse Practitioner licensing board.
• 1+ years of clinical nursing experience.
• Active DEA certification if state allows prescription of controlled substances by APPs.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs, document findings, and collaborate with physicians and the interdisciplinary care team.
• Provide bedside palliative care, education, and support to patients and families during crises.
• Triage patient needs based on acuity and help prevent avoidable hospitalizations.
