Job summary

Enhabit Home Health & Hospice is seeking a Nurse Practitioner to join the interdisciplinary hospice team. Provide medical care to hospice patients, determine eligibility, and manage pain and symptoms as part of the collaborative care plan.

The role requires NP licensure in the practicing state, knowledge of Medicare conditions of participation, and strong technology skills with mobile devices; valid driver's license and reliable transportation are also expected.

Qualifications

• Must be a graduate of an approved school and licensed as a nurse practitioner (NP) in the state where they practice.
• Education and experience are preferred for hospice roles.
• Must have regulatory knowledge, including Medicare conditions of participation.

Responsibilities

• Provide medical care to hospice patients.
• Determine hospice eligibility and pain and symptom management.
• Participate in the care of the hospice patient as a member of the interdisciplinary team.
