Job summary

Elara Caring in Michigan seeks a Hospice Nurse Practitioner to lead in-home palliative care and hospice services. You will conduct admissions, transfers and recertifications, assess patient needs, document findings, and collaborate with physicians to manage symptoms and the Plan of Care.

Qualifications

• Current NP license in the state of practice.
• Master’s degree in nursing with NP certification.
• 1+ year of clinical nursing experience.
• DEA certification if allowed to prescribe controlled substances.

Responsibilities

• Complete hospice visits including admissions, transfers and recertifications.
• Assess patient needs, document findings and coordinate with physicians.
• Provide bedside palliative care and family support during crises.
• Triage needs to prevent avoidable hospitalizations at home.
• Maintain accurate records, billing and CMS-compliant documentation.
• Collaborate with Medical Director and interdisciplinary team.
• Provide staff training and education on hospice care standards.
• Assist in developing clinical practice guidelines.
