Job summary

Elara Caring is seeking a Nurse Practitioner to join our home-care team in Michigan. You will provide high-quality, patient-centered care in patients’ homes, collaborating with physicians, nurses, and support staff to coordinate treatment plans and ensure timely, compassionate care.

The role requires a Master’s in Nursing, active NP licensure, and a valid DEA license, with 3+ years of nursing experience and hospice/palliative exposure.

Qualifications

• Master's Degree in Nursing and licensure as a Nurse Practitioner.
• 3+ years clinical nursing experience.
• 1+ year post-graduate clinical nursing experience in hospice/palliative care.
• Active DEA license.
• Willing to travel up to 50% as needed.

Responsibilities

• Ensures activities align with the vision of Elara Caring’s board, executive team, and Hospice leadership.
• Prevents unwanted hospitalizations by meeting patient and family needs in the home.
• Prepares and maintains accurate patient records, charts, and documents for sound medical practice and reimbursement.
• Complies with applicable laws and regulations regarding Collaborative Agreements.
• Performs billing and coding for patient encounters according to CMS guidelines.
• Provides training and continuing education for staff.
• Assists in development of clinical practice guidelines for quality palliative care.
• Adheres to patient confidentiality for patients, families, staff and the organization.
• Collaborates with Medical Director, attending physician, branch leadership and IDG.
• Performs other duties/projects as assigned.
