Job summary

Elara Caring is seeking a Nurse Practitioner to join our home-based care team serving patients in New York. The role emphasizes comprehensive patient care, care coordination, and adherence to CMS guidelines.

The NP will work under Hospice leadership, maintaining detailed patient records, supporting clinical guidelines, and collaborating with Medical Director and branch leadership. Travel up to 50% is required.

Qualifications

• Masters Degree in Nursing and NP licensure.
• 3+ years clinical nursing experience.
• 1+ year post-graduate hospice/palliative care experience.
• Family Medicine, Geriatrics or psychiatry experience preferred.
• Active DEA license.
• Willingness to travel up to 50%.

Responsibilities

• Align activities with Elara Caring’s board, executive team, and Hospice leadership.
• Prevent hospitalizations by addressing patient and family needs at home.
• Maintain accurate patient records to support medical practice and reimbursement.
• Comply with laws regarding Collaborative Agreements.
• Complete billing and coding per CMS guidelines and initiate reimbursements.
• Provide staff training and continuing education.
• Support development of clinical practice guidelines for quality palliative care.
• Maintain confidentiality of patients, families, staff and organization.
• Collaborate with Medical Director, attending physician, branch leadership and IDG.
• Performs other duties as assigned.
