Job summary

Affinity Health Management in New Jersey seeks a Nurse Practitioner to deliver comprehensive hospice services under physician supervision across Sussex, Morris, Union, Bergen, Essex, and Passaic counties. You will evaluate health history, plan care, and coordinate home-based hospice interventions with the interdisciplinary team.

Requirements include NP with Master’s, state licensure, DEA where applicable, and CHPN/Hospice certification preferred.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license, if applicable to state regulations.
• Current nursing licensure in State and CPR certification.
• Master's degree with a minimum of one year Nurse Practitioner experience and a minimum of one year hospice or palliative care experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.
• Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN))
• Excellent observation, verbal and written communication skill, problem solving skills; nursing skills per competency checklist.
• Must be able to lift, position and transfer patients; valid driver's license.

Responsibilities

• Provides medical care and manages terminal illness under physician supervision.
• Completes history and physical examinations and documents findings.
• Develops individualized plan of care and coordinates hospice services.
• Completes hospice face-to-face visits to assess eligibility and reports to hospice medical director.
• Makes recommendations regarding patient eligibility for certification/recertification.
• Coordinates care with hospice interdisciplinary team and other practitioners.
• Maintains safe environment and ensures continuity of care.
• Interacts with family/caregivers to address goals of care.
