Qualifications

• Graduation from an accredited School of Nursing and an accredited NP Program.
• Current DEA license, if applicable to state regulations.
• Current nursing licensure in the state and CPR certification.
• Master's degree with at least one year NP experience and hospice/palliative care experience.
• Certification in a specialist area (e.g., CHPN, Pediatrics, Geriatrics).
• Strong communication, problem‑solving, and clinical skills.

Responsibilities

• Provide medical care and manage terminal illness under supervision.
• Perform history and physical exams and document findings.
• Develop and implement an individualized plan of care and coordinate hospice services.
• Complete hospice face-to-face visit requirements for eligibility.
• Collaborate with hospice interdisciplinary team and educate patients and families.
• Maintain accurate clinical documentation.
