Job summary

Affinity Hospice Management LLC is seeking a Nurse Practitioner to provide comprehensive hospice care across Sussex, Morris, and nearby counties. You will operate under a collaborating physician, perform assessments, manage terminal illnesses, and plan individualized care in patients’ homes and communities.

The role emphasizes collaboration with the hospice interdisciplinary team, maintaining patient comfort, and ensuring adherence to Medicare/Medicaid guidelines and quality standards.

Qualifications

• Graduation from an accredited School of Nursing and NP program.
• Current NP licensure and physician supervision.
• Master's degree with at least 1 year NP and hospice/palliative care experience.
• Board certification in Hospice/Palliative care preferred (CHPN).

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history, physicals, order/interpret tests and document findings.
• Develop care plans, prescribe medications, coordinate hospice services.
• Assess hospice eligibility and contribute to recertification decisions.
• Educate patients and families; support continuity of care with the team.
• Maintain HIPAA-compliant documentation and participate in quality improvement.
