Job summary

Affinity Hospice is seeking a Registered Nurse/Case Manager to lead inpatient/home hospice care planning and delivery. The role emphasizes community health education and directing services to meet patient and family needs in homes and communities.

You will coordinate with the interdisciplinary team, manage the patient plan of care, and oversee aides while supporting department goals and on-call duties.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current State license as a Registered Nurse.
• CPR certification is current.
• Minimum two years RN experience in clinical care/home health/hospice preferred.
• Must have insured, drivable vehicle and valid license.
• Excellent observation and communication skills; basic math and nursing competencies.

Responsibilities

• Case manages and directs the interdisciplinary team and family.
• Implement and document the patient plan of care.
• Provide care with OSHA infection control measures.
• Ensure care continuity with proper documentation.
• Monitor cases for payer requirements and compliance.
• Demonstrate commitment to professional growth and agency policies.
• Perform on-call responsibilities as assigned.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties to support department goals.
