Job summary

Apploi seeks a compassionate registered nurse to provide hospice home care across homes and communities in Middlesex, New Jersey. You will assess patients, plan and implement care, educate families, and coordinate with the interdisciplinary team to ensure comfort and dignity at end of life.

Requirements include NJ RN license, CPR, and experience in home health or hospice; a driver’s license is recommended for travel within the service area.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed to practice in New Jersey.
• 1 year of professional nurse experience within last 3 years or a BSN with at least 2 years in public health/home care/hospice.
• Must have valid driver's license and automobile.
• CPR certification required.
• Excellent observation, communication, and problem-solving skills.

Responsibilities

• Perform comprehensive patient and family assessments for hospice needs.
• Provide nursing care following the nursing process.
• Develop and adjust care plans with patient and family.
• Administer medications and treatments per physician orders.
• Educate patients and families on care and self-management.
• Coordinate with interdisciplinary team and act as case manager when assigned.
