Job summary

LifeSource Hospice is seeking a compassionate Registered Nurse Case Manager to join our interdisciplinary team providing patient-centered hospice care in New Jersey. The RN Case Manager coordinates care, conducts comprehensive assessments, and collaborates with physicians and the care team to implement individualized plans of care.

The role requires an active NJ RN license, CPR certification, and at least 1 year of nursing experience, preferably in hospice or home health.

Qualifications

• Current, unrestricted RN license in NJ.
• Graduate of an accredited school of nursing.
• Minimum of one year of professional nursing experience.
• Hospice, home health, palliative care, oncology, geriatrics, or acute-care experience preferred.
• CPR certification.
• Strong clinical assessment, symptom-management, communication, and organizational skills.
• Ability to work independently in patients’ homes while collaborating with an interdisciplinary team.
• Valid driver’s license and reliable transportation.
• Ability to participate in weekend and holiday on-call rotations.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Conduct initial and ongoing comprehensive nursing assessments.
• Develop, implement, and update individualized plans of care in collaboration with patient, family, and interdisciplinary team.
• Provide skilled nursing care focused on symptom management, pain control, comfort, and quality of life.
• Educate patients, families, and caregivers on disease progression and end-of-life care.
• Coordinate medications, supplies, equipment, and services related to the terminal diagnosis.
• Communicate changes in patient condition to physicians and team.
• Make routine, on-call, and crisis visits as needed.
• Participate in interdisciplinary meetings and care planning.
• Maintain timely, complete documentation in the EMR.
• Ensure care complies with CMS Conditions of Participation and regulations.
• Support patients and families through end-of-life care
• Provide handoff communication for on-call coverage
• Participate in quality improvement and infection control activities
• Perform pronouncements and post-mortem care when required
• Demonstrate sensitivity to values, culture, and end-of-life wishes
