Job summary

UnitedHealth Group is seeking an RN Case Manager in New Jersey to coordinate individualized hospice care. You will assess patients, collaborate with the interdisciplinary team, and develop care plans tailored to each patient’s needs.

Responsibilities include documenting progress, guiding families, performing on-call duties, and ensuring compliance with policies. A comprehensive benefits package is provided.

Qualifications

• Current RN license valid in the assigned state.
• At least 1 year of nursing experience.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop a plan of care.
• Makes referrals to other hospice disciplines or documents rationale for non-referral.
• Maintains individualized patient care progress records accurately and timely.
• Records pain and symptom management changes and outcomes.
• Teaches patients, families and caregivers appropriate care techniques.
• Communicates regularly with the clinical manager and care team.
• Performs on-call responsibilities as assigned.
• Other duties as assigned.
