Job summary

Virtua Health is seeking a Hospice RN Case Manager to provide compassionate, in-home hospice care across New Jersey. You will assess, plan, and implement individualized care, educate families on symptom management, and coordinate with physicians and an interdisciplinary team to ensure comfort and quality of life for terminally ill patients.

The role requires 2–3 years of hospice experience, NJ RN license, CPR/BLS, and a valid driver’s license.

Qualifications

• Minimum of 2–3 years hospice nursing experience.
• Current State of New Jersey RN license to practice as an RN.
• CPR/BLS certification.
• BSN preferred.
• Must possess a valid driver’s license and auto insurance.

Responsibilities

• Assess patients in their home and implement individualized hospice care plans.
• Provide nursing care, education, and emotional support to patients and families.
• Administer medications and treatments per the plan of care.
• Document care and communicate changes to the physician and interdisciplinary team.
• Participate in on-call, weekend, and holiday coverage as needed.
