Job summary

Affinity Hospice Management LLC is seeking an MSW to implement standards of care for medical social work services in hospice for patients and families in New York. As a core Interdisciplinary Group member, you will provide psychosocial support based on assessments and an individualized plan of care.

Responsibilities include psychosocial assessments, family counseling, care planning participation, education on advanced directives, and crisis intervention as needed. On-call duties may apply.

Qualifications

• Master's degree in Social Work (MSW) from CSWE-accredited program.
• Minimum 1 year healthcare/hospice experience preferred.
• Excellent verbal and written communication.
• Licensed driver with reliable vehicle.
• CPR certification required.

Responsibilities

• Assesses psychosocial status of patients and families; communicates findings to the team.
• Counsels patients and families; provides crisis intervention when needed.
• Educates on culture and advanced directives; participates in care planning.
• Maintains accurate clinical notes and coordination with the Interdisciplinary Group.
• Assists with planning for funeral, financial and health care decision responsibilities.
