Job summary

Apploi is seeking a Medical Social Worker (MSW) to implement standards of care for hospice patients and families. You will be part of the Interdisciplinary Group, providing psychosocial support based on assessments, goals, and individualized plans of care.

You will assess needs, educate families on advanced directives, coordinate with physicians, and participate in care planning. Prior experience in hospice and CPR certification are preferred.

Qualifications

• MSW from CSWE-accredited program is required.
• Strong communication and documentation skills are essential.
• Experience with hospice care and crisis intervention is preferred.
• Licensed driver with reliable transportation.
• CPR certification and clean background checks as required.

Responsibilities

• Assess psychosocial status of patients and families related to end-of-life care.
• Conduct social evaluations and plan interventions with the interdisciplinary team.
• Educate on advanced directives and cultural considerations.
• Participate in development of the plan of care and attend interdisciplinary meetings.
• Provide crisis intervention and coordinate with community resources.
• Maintain accurate clinical notes and communicate changes to the care team.
• Assist with funeral planning, financial, legal, and healthcare decision support.
